Module and test docstrings state what is pinned without the record number or step that introduced it. Incident accounts become the failure shape in the present tense: the pipefail head, the code_shapes import, the export order, the facet dialects, the cross-kind repeat, the write-once kind. bvandeusenandClaude Opus 5.5	be62abf142	perf(retrieval): one embedding per query per plugin request (milestone 456 step 1, #4903) ... A single plugin request fans one query out to several ranked arms. The operator message is searched by auto_inject, its reuse and lesson slots, prompt_rule, the preference slot and rule_via_lesson, and each arm called get_embedding on its own: up to six model calls for one vector. - embeddings.query_embedding_memo(): a request-scoped ContextVar memo that get_embedding consults. Outside a scope the default is None, so every other caller is unchanged. A failed embedding is not remembered. - memoized_query_embeddings decorates /retrieve, /tool-rules and /prior-art. - get_embeddings (document chunks) is untouched. bvandeusenandClaude Opus 4.8	b255a0f90e	refactor: rename package fabledassistant -> scribe (code-only) ... Renames src/fabledassistant -> src/scribe and all imports, plus the default DB name and DB user/password (fabled -> scribe) in config + compose. 952 refs / 154 files. Reverses the old 'internal name stays fabledassistant' convention. Code-only: live databases are still physically named 'fabledassistant'. Deployed environments must set POSTGRES_DB / POSTGRES_USER (or rename the DB) since the defaults now resolve to 'scribe'. Repo (FabledScribe), git host (fabledsword), MCP (fabled-git) and the image name (fabledscribe) are intentionally unchanged. ruff check src/ clean locally; CI (typecheck + pytest) is the gate. Replaces the Ollama HTTP get_embedding with a fastembed.TextEmbedding singleton loaded lazily on first call. Model: BAAI/bge-small-en-v1.5 (384-dim), cached to /data/fastembed-cache. Public API unchanged: - get_embedding(text, model=None) — `model` now silently ignored - upsert_note_embedding - semantic_search_notes - backfill_note_embeddings _cosine_similarity gains a defensive length-mismatch check so any stale 768-dim row that survived the migration is treated as 0.0 similarity rather than crashing zip(). The Ollama client dep stays in pyproject for now (other services still use it); Phase 7 removes it once chat/journal/curator are gone.
