With no SECRET_KEY set, Grimoire fell back to a fixed placeholder. The
quickstart compose substituted change-me-in-production, so the README's
own update step (pull, up -d) recreated the container on that placeholder,
and every credential sealed under the key the user started with stopped
opening.
- config: with neither SECRET_KEY nor SECRET_KEY_FILE, the first start
writes a random key to DATA_DIR/secret_key (mode 0600, published with a
hard link so concurrent starts agree), and later starts read it back.
An explicit key wins and leaves the file alone. A missing
SECRET_KEY_FILE or an empty key file stops the start instead of
switching keys. With no data directory (a run outside the container)
the process gets a temporary key and says so.
- app: warns on a public placeholder key, on a temporary key, and when
/data is not a mounted volume.
- Dockerfile creates /data.
- docker-compose.yml is now the deploy compose (published image, both
volumes, nothing to set). The build-from-source compose is
docker-compose.dev.yml, and docker-compose.quickstart.yml is gone. The
Swarm stack file gains the /data volume and keeps its explicit key,
because a Swarm can move the app away from a node-local volume.
- README: a Deploy section with the compose inline, SECRET_KEY optional,
"keep both volumes", and updates that keep the key. A test holds the
README's copy equal to the file.
- Installs that ran on the change-me-in-production placeholder get a key
of their own on the new compose; Settings then asks once for saved
forge tokens and the backup secret.

Step 2 of the Scribe -> Grimoire rename.
- src/scribe -> src/grimoire, and every import, patch target, module path,
hypercorn entry point and `python -m grimoire.restore` with it; pyproject,
uv.lock and the frontend package are named grimoire.
- Image git.fabledsword.com/bvandeusen/fabledgrimoire; CI's Postgres service
runs as grimoire / grimoire_test.
- Compose files default the DB user, database and (prod) volume to grimoire,
each overridable from .env, so an install created as Scribe keeps its data
by setting POSTGRES_USER, POSTGRES_DB and PGDATA_VOLUME.
- Backups are written as grimoire-<ts>.tar with format grimoire-backup-tar.
Archives named scribe-* with the scribe-backup-tar manifest still restore
and are still counted at the destination (folder and S3), with tests.
- Exports, the MCP auth realm, the ASGI scope key and the persistent logger
carry the new name.
Kept on purpose: the secret-sealing key label stays "scribe.secrets.v1",
because every stored secret was derived under it.
Plugin identity (plugin name, hooks, env vars, .scribe marker, skills) is
step 3.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Step 1 of the Scribe -> Grimoire rename: every capitalised "Scribe" in prose
becomes "Grimoire" ("Fabled Scribe" -> "Fabled Grimoire") across the UI,
the PWA manifest and page title, the MCP instructions and tool docstrings,
hook and skill text, docs and README, with the tests that assert on that
prose moving with it. Old migrations keep their wording.
Identifiers are untouched here: the package, plugin name, MCP server key,
hook file names, env vars, the .scribe marker and FabledScribe repo/image
names move in steps 2 and 3, along with prose that quotes them.
The sweep matched "Scribe" only as a whole word, so describe/subscribe and
friends are untouched. The longer name took the MCP instructions one
character over their 1,600 budget; two phrases are shortened to fit.

Brings the three compose files in line with rule 198 (milestone 544,
step 6203, at the operator's request):
- no healthcheck on the app or the database, so a slow pg_isready on
stalled storage can no longer get a working database killed;
- no deploy.restart_policy in the Swarm file, so Swarm's default
(condition: any) restarts clean exits too, instead of on-failure with
max_attempts leaving the app down;
- depends_on is plain start order; restart: unless-stopped stays on the
plain-Compose files (Swarm ignores it), and the dev app gains it.
The orchestrator used to be what retried a container that started before
Postgres was ready. That is now the app's job: alembic/env.py calls
services/db_ready.wait_for_database before migrating. It retries refused
connections, unresolved hosts, connect timeouts and "the database system
is starting up" with 1-2-4-...-10s spacing for up to 300s, then exits
non-zero; a wrong password or missing database fails at once.
Per-request recovery is the engine's pool_pre_ping.
Also fixes the dev compose database image: postgres:16-alpine has no
vector extension, so migrations could not run against it. It now uses
pgvector/pgvector:pg17 like the other two files.

Comment-only pilot for the comment standard (rule 32, milestone 544).
Issue and rule numbers, dates, run ids and incident stories come out;
the constraint each one guarded stays as a present-tense sentence.
History removed from the files, kept here:
- Dockerfile/CI: installing from uv.lock replaced pip/uv-pip installs that
ignored the lock (#2194, the mcp 2.0.0 break on 2026-07-28); --locked
then caught pgvector missing from the lock.
- CI: plugin paths were added after #2198/#2209 reached a live install;
`integration` joined the build's needs after run 8212 published
:latest with integration red (#4958); the `[[ ]]` tag step failed
silently on dash (2a374d9); BUILD_VERSION carried the channel until
2026-08-31 (#3298).
- jq was removed from the hooks in #4107.
- prod compose: the lenient DB healthcheck followed a runc setns crash
loop; pgvector arrived with migration 0067 via a PG16->17 dump/restore.
Corrections to match the code: README names the Settings tab
"MCP Access"; ci-requirements says the Dockerfile builds on node:22
and lint covers scripts/.

Move semantic_search_notes off the full-table Python cosine scan onto a native
pgvector column: indexed ORDER BY embedding <=> :q LIMIT k (HNSW, cosine).
Migration 0067 enables the extension, converts the JSONB embedding column to
vector(384) (stale-dim rows dropped and regenerated by the startup backfill),
and builds the HNSW cosine index. Postgres image moves postgres:16-alpine ->
pgvector/pgvector:pg17 across prod, quickstart, and CI.
Scribe: project 2, milestone 93, task 1031.

Renames src/fabledassistant -> src/scribe and all imports, plus the
default DB name and DB user/password (fabled -> scribe) in config +
compose. 952 refs / 154 files. Reverses the old 'internal name stays
fabledassistant' convention.
Code-only: live databases are still physically named 'fabledassistant'.
Deployed environments must set POSTGRES_DB / POSTGRES_USER (or rename the
DB) since the defaults now resolve to 'scribe'. Repo (FabledScribe), git
host (fabledsword), MCP (fabled-git) and the image name (fabledscribe)
are intentionally unchanged.
ruff check src/ clean locally; CI (typecheck + pytest) is the gate.

Drift-audit Group 7 (renamed/removed lingers) + Group 5 #9:
- docker-compose.prod.yml pulled fabledassistant:latest, a tag CI stopped
publishing after the rename. Point it at fabledscribe:latest (the name CI
and quickstart use). The internal DB name stays fabledassistant by design.
- Remove the unused hard-delete delete_note imports from the notes and tasks
route modules (they delete via trash; the import was an attractive nuisance
that bypassed soft-delete).
- delete_rule MCP tool: docstring/warning said 'permanently delete' but the
body moves the rule to recoverable trash. Corrected to match.
- Delete services/calendar_sync.py: fully orphaned (zero importers) and it
read Config attrs that no longer exist, so any re-wiring would crash.
- Remove dead services: notes.search_notes_for_context and logging.log_generation
(zero callers; log_generation wrote a 'generation' category no stats/UI surface).

Mitigation for the nightly fabledscribe Postgres outage on the
vdnt-docker02 Swarm node (incidents 2026-05-15/16/17 around 03:50 UTC).
Confirmed kill chain (not the trigger): a brief host-level setns/exec
stall makes the Docker healthcheck exec fail with exit 1 → unhealthy →
SIGKILL → fast-shutdown can't finish on NFS in 10s → exit 137 → swarm
restart_policy.max_attempts: 5 burns out → DB stays dead.
Hardens the `db` service so a transient host blip can't escalate to
killing the database:
- stop_grace_period: 120s (gives PG room to fsync on shutdown)
- healthcheck: interval 30s / timeout 10s / retries 10 / start_period 180s
(only gates app startup order — not authoritative liveness)
- prod: restart_policy condition=on-failure, max_attempts=0, window=120s
- quickstart/dev: restart: unless-stopped
Host-side trigger (what stalls runc/exec at ~03:50 UTC) is still under
investigation — see project_pg_nightly_outage.md.
Note: the Portainer prod stack differs from docker-compose.prod.yml
here (NFS bind, traefik labels, no ollama). The same `db:` block needs
to be pasted into Portainer for the prod mitigation to apply.

- TRUST_PROXY_HEADERS=true: ensures rate limiting keys on the real client IP
(X-Forwarded-For / X-Real-IP) rather than the Traefik container IP
- SECURE_COOKIES=true: sets the Secure flag on session cookies since the app
is served behind TLS via Traefik
- Remove ports: "5000:5000" — Traefik routes traffic internally; publishing
the port directly would bypass Traefik middleware

Docker Compose:
- Enable Ollama GPU passthrough (nvidia, count: all) in both dev and prod files
- Add OLLAMA_FLASH_ATTENTION=1 (faster attention on GPU in both files)
- Add OLLAMA_MAX_LOADED_MODELS=2 and OLLAMA_KEEP_ALIVE=30m to prod (was already in dev)
- Remove 8G memory limit from prod Ollama service (CPU-bound constraint, no longer valid)
llm.py:
- Increase num_ctx 16384 → 32768 in stream_chat and stream_chat_with_tools (GPU VRAM allows it)
- Increase num_predict cap 4096 → 8192 for tool-augmented responses
generation_task.py:
- Parallelize build_context, get_tools_for_user, and get_setting all from the start
- As soon as tools list is ready (fast DB call), launch classify_intent as an asyncio.Task
- Await build_context and classify_intent together via asyncio.gather
- Intent result is pre-computed before the generation loop; loop just reads pre_intent on round 0
- intent_ms timing now reflects wall-clock time from intent start to completion
